Service Contract-C

Care Manager and family work together to create and implement a 6 month contract covering a specific scope of work to be performed. See Care Management Time and Travel Time sections below for fees charged for services rendered under the contract. At the end of the six-month period, a Service Contract may be renewed one time with payment of a new fee or converted to a growth plan. In the event the contract is converted to a growth plan.
